In order of ascending difficulty, it's Maths Studies SL, then Maths SL, Maths HL, and then a super hard HL level of maths that virtually no-one ever does.

I'm pretty sure that's not the case. They didn't get combined. 99.95 as the highest mark is more logical because a percentile is defined as the percentage of people who got below you. Thus it's technically impossible to get 100.
