I am doing 5.3 maths in year 10 right now , what is the main difference between Advanced and Extension 1 Maths like is it much harder than Advanced as in the concepts and topics?

Yes, extension 1 maths is harder than advanced. Advanced maths and standard maths now have a common section as well. If you're doing well in 5.3 maths then you should probably give extension 1 a try.
