So, what you do, is, under "plaintiff's name, address, etc" you put
'Mrs Jones' and make up her address, etc.
Under Defendent's name, you put Randwick Council. According to her in the lecture we've gotta look up their address or we lose marks

Plead the cause of action - I think this is where you put in the details about why they've been negligent... something along the lines of 'the council told me it was safe to swim but I bumped my head and hurt myself'... but I don't know how that fits into the "1}" and "2}" bit...
...particulars of injury: fractured skull, pain and suffering, economic loss (lost wages)...

Edit: Right. Just listened to my recording of her attempt at explaining it. However, from memory she was actually using a different form to explain it to the one we have to fill out, and then found the original and got all confused and confusing. Grr she's a bad lecturer. Anyone remember when question time is? I'll go along and ask them what the go is...
