whats the difference?
is it that conciliation is more formal with an tribunal ie IRC and mediation is just with any 3rd party??

yes...from least to most formal:
negotiation
mediation
conciliation
arbitration

negotiation is like 2 people sitting in an office talking about what each wants
mediation is just when a third party is present to basically make sure fisty-cuffs dont break out
conciliation is the same as mediation, but the neutral third party participates in the process
arbitration is by far the most severe and takes place in courts
