if you receive an offer for a course and defer, does it mean that a vacancy opens up in the course?

they actually overallocate thier offers already in anticipation of x% not accepting, which they can roughly estimate from past experience, and its consistant from year to year.

e.g there are 200 places, but they offer about 210 places in main round, with the anticipation that 5% wont accept or defer
