Yeah something like this should be quite formal.

Perhaps some other questions could include:

"How did/would you deliver excellent service for the job?" - Can be for both Staff/Manager

"Do you find this job to have a nice working atmosphere?

"Are there any disputes between staff members?"

etc..

"whats the worste experience you've had?"

might be changed to:

"Have you experienced any undesireable incidents during your employment period?"

Other questions:

1) What is the policy on grievance procedures at work? Are they easily accessible and readily available?

2) What sort of culture does the workplace foster?

3) Does your income compare favourbly amongst other employees of similar profession/status?

Something like that.

instead of asking exact wage, like what sportsgal said, but maybe provide wage brackets
ie. less than $xxx, between $xxx - $xxxx, more than $xxxx

also when asking wages remeber to provide ages aswell.
especially for staff, and if its sizzlers, rates would change between ages so include that
