Yeah, that's right. And yes, you do need to write an address and date for a letter as the form of one requires it.

A word of advice, don't listen to uneducated people such as the above ^, he had no understanding of the english language and does standard english.
To answer your question, the date may simply be the day you have done the exam, the address is not important, however the persona you are writing to is.

The more important focus of your letter should be the content, the what and the how. If its a creative peace, than you make motifs, symbols and techniques. However if it is an analytical piece than you mention the techniques and its effect in the form of the letter. The structure is important to getting into the band 6 range, however you may still achieve a band five by writing the content.

Edit- As for english in general, it is subjective, you may well get good marks just because of your name. Teachers mark like this and your name may be so good that you may write total garbage and get full marks, I have actually seen evidence of this, one person couldn't even put a proper sentence together, "It give good insight."

Also, in my last assessment all 5 english teachers had conflicting thoughts on how to do it. For instance, length is a paragraph each scene and another said a few lines, one said we could type it, the other said you couldn't... the list gave me headaches, it was the hardest one I did because of these factors alone.

Well good luck.
