HEy!... I need more people to take my questionnaire..... if u unda stand my desperate situation or just feel sowy for me...pls click on the link below......dun worry it wont take that long n u myt even enjoy it........

n u alss myt like to use the website I used for your own questionnaire too...coz it graphs everything up soo nicely...makes making surveys soo easy! I definately recommend this site for making PIP questionnaires!!!

I know what you mean about desperate situations!! My CM is due (draft) in less than 3 weeks!!! - I feel your pain!

I've just looked at your questionaire...

Just some sugestions:

- If you arn't going to use the participants personal information - umm - y are you asking for it?

- And if the participant answers "no" to the question..."Does your school offer a reward system?" (or question along those lines) shouldn't there be some sort of direction for them.....something like...."If no, go to question such and such..." if there are anymore questions requiring their imput...OR...(If no) "The survey has now ended...thank you for your co-operation".....

That way they would not be left wondering "Where do i go from here?"

Other than that - Great layout! I'll be sure to use that site for some of my surveys/questionaires!
