Usually it's an exam that makes me study. But the need to be sucessful in the future and one day looking back and not regretting that you didn't try hard enough or you could have done better. Beating friends is always a motivation as well. Hahahahaha.

I have one of those notebooks that are divided into like 5 subjects. I use class notes and follow the syllabus and write summaries. Remember that whatever you write you should be able to/want to read. And Diagrams/pictures/tables help.
