Having done 2U and 3U I wasn't nervous at all in my job interview - I just thought to myself "Hey! It's in English! How hard can it be? :p" I think most of the good advice has already been said - you need to be spontaneous but also have a core of solid sentences and delaying phrases to fall...

Extension isn't interactive, it's basically a prepared speech - get your general points on poverty etc ready and spit them out inthe most appropriate order once you're inthe room :).

Define the postitive direction yourself at the beginning of the question. Sometimes it's easier to do it one way than the other, so be prepared to change if necessary :).

I think you're exaggerating a little - some questions are out of date, some are not. There is more to doing textbook questions that preparing for exams - some are written specifically to reinforce your understanding of the material, so they may be primarily learning tools rather than assessment...
