If you own IPT, then scaling won't affect you in any real sense. As for your question, if you do 11 units, and a 2 unit subject is your worst subject, then they'll take 1 unit of that subject and count it along with your extension 1 maths unit.

You won't get a mark in 2 unit maths. Simple. Extension 1 and 2 maths counts for 2 units each when you do extension 2 maths, no need for a 2 unit mark. And they don't assume you will get 100% in the 2 unit paper either.

Microsoft Word 2003?? Go to Insert -> Reference -> Footnote, click on Insert and then type in the footnote at the bottom of the page. That's referencing. I think appendices are something else though, look at this page for that info.
