Cover sheet for Humanities? (1 Viewer)

Hey party people,
I have an essay due Tuesday that a friend is taking in for me (I'm only at uni Wednesday & Thursday).
In a moment of supreme blondeness last week, I forgot to pick up a Humanities cover sheet from W6A last week so I could sign it & have it ready to be handed in.
Does anybody happen to have one in pdf form they could email to me? I've looked around on the Department's site and haven't found anything but thought I'd give it a go here as well.

Haha, its the "points" system for ranking and determining those "researching" staff in humanities
