I know you have to put your name/tute time, etc...but do you just write your questions in bullet form or what?

Then in the body, I put the tutors name. Then leave a space and ask a question. Then thanks with you name. Never knew that you had to put the tute time in. But you can do it in bullet points or sentences. I just use sentences because its easier.
