have you had your teacher check through this? it still seems a little unresolved. many questions (eg #6, 8) are very subjective and probably will be biased (ie. little objective results gained)

it's usually easier to have boxes to tick, rather than circling answers. don't forget to have lines for people to write extended answers on -- also add something like "any other comments?" as people invariably have their own opnion that can't be voiced elsewhere.

i think you need to move away from having such open questions (ie. you leavve space for people to give their answers rather than providing a list with "other" as an option) these "open" questions are generally harder to analyse and may act more like qualitative than quantitative questions/answers.

but yes, my general theory is: get this fully checked with teacher before handing out. if it's for an assessment and the teacher won't check it, try a diff one (even if you get a mate from another school to give it to their teacher lol)
