I have done 2 so far and some questions are insane.

i dont do 4U, but i guess the obvious answer to your question, is practise....

maybe go through some of the papers like you would a textbook for a bit, so not under exam conditions, not timed etc, so you can get used to the question and understand how to answer it. see if there are any patterns in how to answer questions, and write that out and make sure you use that. You need to go through as many as you can so you can get the most experience with different types of questions. Then, start timing yourself again and work on your speed and accuracy. In the test, i have always learnt to look at a question for a minute or so and if i know its out of my league at the moment i will skip it and move onto the next one, and will go back to it if i have time. Also make sure to look at the weighting of the question - and try and answer the big markers to get maximum marks. Im not sure if this is what other 4U students suggest though, so if I am wrong in my advice someone please feel free to correct me.

What can I do?

4u papers are known to be notorious but you should be able to get the feel of them after maybe three or four. If your still struggling, then it's a sign that you don't fully understand a concept or topic properly and thus you should go back and revise them, doing challenging exercise from cambridge, fitzpatrick etc.
If you look at the questions you struggle with in each paper, you will notice that each question is related to the same concept and topic, which links to my point above. I find that i struggle the most with mechanics and harder 3u type questions and so i have to continuously go back to the textbook and look over something.The key is to grasp the knowledge before doing practice.
Just do a lot more practice, under exam conditions, and mark your answers afterwards to learn from your mistakes. Worst case scenario, you can ask your teacher, tutor or even post the question on BOS.
Im sure some of the senior members can give you a better insight

I had trouble with last Q's in 4u until this week, when I learnt binomial theorem (3u). Made some of those last questions much much simpler...
