Look at the first to second lines. On the denominator, you factorised 6a-3 as 3(a-1) when it should be 3(2a-1)

change the b^2+2b to b(b+2) and change the 6a-3 to 3(2a-1)

The mistake you have made is you have changed the 6a-3 to 3(a-1) for some reason

You're not bad at math: everyone makes silly mistakes. I make stupid mistakes all the time and I end up staring at the textbook answer and my answer over and over and seem convinced the textbook answer is wrong. Then I show the answer to the teacher the next day and I notice my mistake immediately - fresh eyes make all the difference.

At least you didn't get to that stage :p

The case of exam jitters, I've got them too.

All year - Topping exams, hard questions being smashed...
2 weeks before the exam - I'm differentiating wrong, forgetting +C in integration, mixing up Simpsons Rule and Trapezoidal. Just relax, calm down and focus on what you're writing. I've started and that's decreased a majority of my mistakes.
