I just want to ask you guys particularly physics students if you guys had to do a practical assessment task for the year 11 physics module

Nope. No Prac Assessments at all for me for the World Communicates MODULE.

All I remember was that the experiment involved shining a beam of light on a rectangular glass piece at various angles to determine the angle of refraction, and there followed by questions involving snell's law, drawing wave fronts passing through a denser medium, drawing waves (e.g. with amplitude x and time Y), application of reflection.

Yeah, we did this, and did an experiment with slinkies to determine certain properties of waves.
