Vaccination history record card? (1 Viewer)

This is for Pharmacy students but I guess med students have to do it too--vaccinations! How do you fill in the vaccination history record card and when? Should I just take it to the local GP or do you think my faculty will help us out? (they haven't said anything yet about the vaccination forms). And do you have to fill it in before your first hospital placement and hand it in to your faculty, or to the hospital?

If you are talking about a red and white 'booklet' card, then i can probably help you.

Basically go to your GP and if you don't have the records of your vaccinations, you should give you a blood test.

I'm not sure what happens after that, but basically they analyse what vacinnations you have had and they give you those vaccinations.
