I do ancient and not modern so don’t take this as a gospel, but have a few pieces of key evidence for each syllabus point including your sources as opposed to writing entire paragraphs
Use others as a point of reference and see what they do to address the question. Then, maybe time yourself for 5 minutes using a practice question and see what you’d have for each paragraph, and what evidence you’d include for it
i would suggest making ur own cause it sticks in ur head better, but u said ur not good at the subject so maybe model ur own plans on whatever other people have written.

i found that i could prepare evidence (quotes, events, stats etc) for paragraphs that corresponded to each dot point, and for the essays i could mix and match paragraphs from dot points to shorten how much i have to remember
honestly after finding evidence everything just comes together if i sit and stare at it long enough
