Your Enrolment Package
This package contains all the forms and information you will need to be enrolled at UWS.
Your package should contain the following:
* An Enrolment form (White and Green copy)
* A Student Declaration form
* A Compulsory Student Data and Consent form
* Request for Commonwealth Support
* Commonwealth Support Booklet 2005
* Academic Credit form
* Enrolment Amendment form
* UWS Calendar (University Rules and Regulations)
If you have not received all of the above mentioned forms, you will need to contact 02 9852 5155 to have the additional information posted to you. You can also access the Change of Name form and Academic Credit Application form from the UWS Forms page.
Forms to Complete
To be enrolled as a student at UWS you must ensure that the following Compulsory Forms are correctly completed and returned to the address below no later than Friday 18 February or your offer will lapse.
* Enrolment Form - White Copy ONLY. (The green copy is your copy and should be retained for future reference)
This form lists your personal details, the course, and all units in which you have been pre-enrolled for 2005. Please check this form carefully. Sign and date the form and return with other COMPULSORY forms.
If your personal details are incorrect or incomplete, you will need to complete the enclosed Enrolment Amendment Form and return it with your other forms. If your name details are incorrect, you will need to complete the Change of Name form also available at any Student Centre.
If you need to add or change your enrolled program, refer to the UWS Handbook for your course structure and unit information. Complete the enclosed Enrolment Amendment and return with your other forms.
It is your responsibility to ensure that you are correctly enrolled and that UWS has your correct contact details at all times.
* Student Declaration

* Request for Commonwealth Assistance
Complete all fields and return with other COMPULSORY forms.
Note: If you wish to defer your Student Contribution or Tuition Fee Liability, you MUST provide your Tax File Number.

Complete all fields and return with other COMPULSORY forms.
Optional Forms
The Enrolment Amendment form is optional and is only required to be completed if you need to amend your enrolment form. The Academic Credit Application form should only be completed if you wish to apply for credit for prior learning completed at other tertiary institutions. All other information is provided for your reference and does not need to be returned.
Student ID Card
Your Student ID card can be issued at any UWS Student Centre once your enrolment has been completed. This will provide you with Library borrowing privileges and IT Centre access, and should be carried at all times on Campus.
