Why is "* 7. If yes, What was the cause of the accident?" a compulsory question?

^^^This. Also, you can't categorise all accidents as being caused by one of those five factors. Did it anyway though

Yep. Should've been able to pick multiple choices for that.

I think there should be other categories for some of these Qs-driver inexperience is what I wanted to put down for the one about knowing someone involved in an accident. Also, an other option would be nice for the biggest influence-I think who teaches learners to drive heavily shapes what kind of driver they end up being.

Also do you believe passengers are a distraction whilst driving? I think they potentially can be. But not in the way you'd expect, see my most distracting passenger is my 90 year old nan who will grab at my arms when I drive and that, not the under 25 rule on the red P plates at the certain times. I mean the most distracting passenger under 25 I've ever had in my car was my cousin who'd pester me for lifts when we'd finish early on sport afternoon, not 11PM-5AM.

Filled it in, question 6/7 was pretty weird how it didn't distinguish between us being in an accident and hearing about one. How come for question 7 it assumes the p plater is at fault though?

Quick squizz at the survey, Q7 is too vague.

Ive been involved in numerous accidents where I was not at fault.
